Background:

Distinguishing ovarian carcinoma metastasis from primary breast cancer is crucial for appropriate treatment.
Ovarian serous carcinoma can present with rare metastatic patterns.

Observation:

A patient with stage IIIC ovarian serous carcinoma developed bilateral axillary lymph node metastasis 25 months after initial diagnosis.
Elevated serum CA-125 levels raised suspicion; CT scans showed no abdominal disease, and mammograms were normal.
Bilateral axillary lymphadenopathy was identified and investigated.

Findings:

Fine needle aspiration cytology (FNAC) and biopsy confirmed poorly differentiated carcinoma in the axillary nodes, consistent with the primary ovarian tumor.
This confirmed a rare metastatic event of ovarian carcinoma to the axillary lymph nodes.

Implications:

This case underscores the importance of considering rare metastatic sites for ovarian cancer.
FNAC is a valuable first-line diagnostic tool for rapid confirmation of metastatic disease in unusual locations.
