Background:

  • Distinguishing ovarian carcinoma metastasis from primary breast cancer is crucial for appropriate treatment.
  • Ovarian serous carcinoma can present with rare metastatic patterns.

Observation:

  • A patient with stage IIIC ovarian serous carcinoma developed bilateral axillary lymph node metastasis 25 months after initial diagnosis.
  • Elevated serum CA-125 levels raised suspicion; CT scans showed no abdominal disease, and mammograms were normal.
  • Bilateral axillary lymphadenopathy was identified and investigated.

Findings:

  • Fine needle aspiration cytology (FNAC) and biopsy confirmed poorly differentiated carcinoma in the axillary nodes, consistent with the primary ovarian tumor.
  • This confirmed a rare metastatic event of ovarian carcinoma to the axillary lymph nodes.

Implications:

  • This case underscores the importance of considering rare metastatic sites for ovarian cancer.
  • FNAC is a valuable first-line diagnostic tool for rapid confirmation of metastatic disease in unusual locations.
