Purpose of the Study:

  • To investigate the effect of rosuvastatin on endothelial dysfunction (ED) and inflammatory disease activity in AS patients.
  • To assess rosuvastatin's impact on inflammatory markers, cytokines, lipids, and adhesion molecules.

Main Methods:

  • A 24-week, single-blind, placebo-controlled study involving 32 AS patients.
  • Patients received either rosuvastatin (10 mg/day) or placebo as an adjunct to existing treatments.
  • Flow-mediated dilatation (FMD) assessed ED; inflammatory markers (BASDAI, BASFI, CRP, ESR), cytokines (TNF-α, IL-6, IL-1), lipids, and adhesion molecules (ICAM-1, VCAM-1) were measured.

Main Results:

  • Rosuvastatin significantly improved FMD (p < 0.01) and reduced inflammatory markers, TNF-α, IL-6, and ICAM-1 (p < 0.01).
  • Positive effects on lipid spectrum were observed with rosuvastatin.
  • Significant negative correlations found between FMD and IL-6, ICAM-1, and CRP post-treatment.

Conclusions:

  • Rosuvastatin improves inflammatory disease activity and endothelial dysfunction in AS.
  • It lowers pro-inflammatory cytokines, downregulates adhesion molecules and CRP, thereby improving ED.
  • Rosuvastatin offers anti-inflammatory effects and modifies vascular risk factors in AS through cholesterol-dependent and -independent pathways.
