Intermittent catheterization (IC) is recommended over continuous catheterization (CC) for laboring women with epidurals. IC did not increase cesarean birth rates, unlike CC, which was linked to a higher likelihood of cesarean birth.

Area of Science:

  • Obstetrics and Gynecology
  • Urology
  • Anesthesiology

Background:

  • Continuous urinary catheterization (CC) is common during labor with epidural anesthesia to prevent urinary retention.
  • Recent evidence questions CC use due to potential urinary tract infection (UTI) risks.
  • This study compares CC with intermittent catheterization (IC) for laboring women.

Purpose of the Study:

  • To evaluate the impact of CC versus IC on the duration of the second stage of labor.
  • To assess the incidence of postpartum UTIs in women undergoing CC versus IC.
  • To determine the effect of different catheterization methods on cesarean birth rates.

Main Methods:

  • A randomized controlled trial was conducted with low-risk, nulliparous women at ≥37 weeks gestation receiving epidurals.

  • Participants were randomized to either CC or IC for bladder management during the first stage of labor.
  • The study included 123 women, with 55 in the CC group and 68 in the IC group.
  • Main Results:

    • No significant differences were observed in the length of the second stage of labor between the CC and IC groups.
    • The overall incidence of UTIs was low in both groups.
    • Women in the CC group had a significantly higher rate of cesarean birth (27.3%) compared to the IC group (10.3%).

    Conclusions:

    • Intermittent catheterization (IC) appears to be the preferred method for bladder management in laboring women with epidurals.
    • Continuous catheterization (CC) was associated with a significantly higher rate of cesarean births.
    • Further research is needed to understand the relationship between continuous urinary catheter use and birth outcomes in laboring women with epidurals.
