    Summary

A special pacemaker effectively treated drug-resistant supraventricular tachycardia in three patients. However, success rates varied for ventricular tachycardia, indicating a need for further development in pacemaker technology for arrhythmia management.

    Area of Science:

    • Cardiology
    • Biomedical Engineering

    Background:

    • Drug-resistant supraventricular tachycardia (SVT) and ventricular tachycardia (VT) pose significant clinical challenges.
    • Pacemaker technology offers potential therapeutic options for managing these arrhythmias.

    Observation:

    • An Omni Orthocor 234A pacemaker was implanted in nine patients (3 with SVT, 6 with VT).
    • The device uses an external unit for stimulation at various intervals (195-500 ms).
    • Noninvasive electrophysiology studies were conducted using a triggered mode responsive to chest wall stimulation.

    Findings:

    • Effective treatment was observed in three patients with reciprocating SVT over a 2.2-year follow-up.
    • Four out of six patients with VT remained successfully managed during a 1.9-year follow-up.
    • The device demonstrated utility in serial noninvasive electrophysiologic testing.

    Implications:

    • This pacemaker technology shows promise for selected patients with specific tachyarrhythmias.
    • Further refinement is needed for broader clinical application in arrhythmia management.
    • The device may aid in long-term clinical follow-up and electrophysiologic testing.
