    Background:

    • Radical cystectomy is the standard treatment for muscle-invasive bladder cancer.

    Purpose of the Study:

    • To analyze preoperative comorbidities, intraoperative, and postoperative complications in patients aged 75 and above undergoing radical cystectomy.

    Main Methods:

    • Retrospective clinical study of 46 patients over 75 years old who underwent radical cystectomy.
    • Data collection included indications for surgery and intraoperative/postoperative complications.

    Main Results:

    • 76% of patients presented with anemia due to hematuria preoperatively.
    • Common urinary diversions included ileal conduit (52%) and ureterocutaneostomy (35%).
    • Average surgery duration was 190 minutes, with 76% requiring blood transfusion.

    Conclusions:

    • Gross hematuria from bladder cancer leading to anemia was the primary indication for cystectomy in this age group.
    • Patients over 75 experienced prolonged hospitalization and a higher incidence of complications following radical cystectomy.
