The lateral view of the cranium is dominated by temporal, sphenoid, and ethmoid bones.

Summary

Inflammatory pseudotumor (IPT) of the lateral skull base is rare and often misdiagnosed. Surgical resection is the most successful treatment, leading to high rates of disease-free patients.

Background:

  • Lateral skull base inflammatory pseudotumor (IPT) is a rare, non-neoplastic condition.
  • Its unknown etiology often leads to misdiagnosis as malignancy or infection.

Purpose of the Study:

  • To systematically review all reported cases of lateral skull base IPT.
  • To provide insights into the management of this uncommon lesion.

Main Methods:

  • Systematic review of MEDLINE/PubMed databases.
  • Analysis of demographics, presentation, imaging, treatment, and outcomes from 30 articles describing 39 patients.

Main Results:

  • Hearing loss was the most common symptom (53.8%); mastoid bone was the most frequent site (61.5%).
  • CT and MRI revealed characteristic findings; histopathology showed fibrosis and inflammation.
  • Surgical resection (92.3%) was the primary treatment, achieving a 96.0% disease-free rate.

Conclusions:

  • Comprehensive workup including clinical exam, imaging, and biopsy is crucial for diagnosing lateral skull base IPT.
  • Surgical excision is the most effective treatment, with surgery combined with corticosteroids as a secondary option.
