Purpose of the Study:

  • To correlate the histology of carotid endarterectomy specimens with clinical data.
  • To identify specific plaque morphologies associated with neurologic deficits.
  • To determine the role of plaque morphology versus luminal narrowing in cerebral ischemia.

Main Methods:

  • Performed 278 carotid thromboendarterectomies from January 1988 to December 1989.
  • Systematically analyzed 250 endarterectomy specimens using light microscopy.
  • Classified plaques as complicated (86%) or simple fibrous (14%) and analyzed pathological features.

Main Results:

  • Complicated plaques (86%) were significantly more symptomatic (53%) than simple plaques (21%).
  • Fresh thrombus (70% symptomatic) and ulceration (66% symptomatic) showed strong correlations with neurologic deficits.
  • Intraplaque hemorrhage and organized hematoma did not show significant correlation with symptoms.

Conclusions:

  • Carotid plaque morphology, particularly the presence of fresh thrombus and ulceration, is a primary determinant of cerebral ischemic events.
  • Plaque characteristics are more critical than luminal narrowing in predicting neurologic deficit.
  • Histopathological analysis of carotid plaques aids in understanding stroke pathogenesis.
