Background:

High-grade primary renal leiomyosarcoma (LMS) is a rare and aggressive renal malignancy.
Understanding its clinical characteristics and prognosis is crucial for patient management.

Purpose of the Study:

To investigate the clinical characteristics, prognosis, survival, and diagnosis of high-grade primary renal leiomyosarcoma.
To correlate findings with existing literature.

Main Methods:

Retrospective review of 10 cases of high-grade primary renal LMS diagnosed between January 2003 and April 2013.
Analysis of clinical manifestations, treatment, and outcomes, with literature correlation.

Main Results:

Ten patients (5 male, 5 female; age 43-77) with tumors ranging from 3-18 cm were analyzed.
40% were asymptomatic; 60% presented with lumbar pain. Nephrectomy was performed in 90% of cases.
Histopathology revealed high-grade pleomorphism and necrosis; immunohistochemistry showed vimentin (100%), desmin (90%), and smooth muscle actin (80%) positivity. CD117 was negative. Mean survival was 23.9 months (range 6-68 months).

Conclusions:

High-grade primary renal LMS is rare, highly malignant, and carries a poor prognosis.
Early diagnosis and radical nephrectomy are vital for improving patient survival.
Surgery remains the primary treatment modality for renal LMS.
