    Background:

    • Age-related cataract is a leading cause of visual impairment.
    • Phacoemulsification with intraocular lens (IOL) implantation is a standard surgical treatment.
    • Advancements in IOL technology aim to restore functional vision at multiple distances.

    Purpose of the Study:

    • To compare early visual function in patients receiving multifocal (ZMA00) versus accommodative (FLEX) intraocular lenses.
    • Evaluate visual acuity and contrast sensitivity outcomes after cataract surgery.

    Main Methods:

    • 112 patients with age-related cataract were randomized into ZMA00 (56 eyes) and FLEX (56 eyes) IOL groups.
    • Phacoemulsification was performed, followed by IOL implantation.
    • Visual acuity and contrast sensitivity were assessed at multiple postoperative time points.

    Main Results:

    • No significant differences in uncorrected distant visual acuity at 1 week, 1, 3, or 6 months post-operation.
    • No significant differences in best-corrected distance and intermediate visual acuity or contrast sensitivity at 6 months.
    • The ZMA00 group showed significantly better near best-corrected visual acuity, reading speed, and spectacle independence compared to the FLEX group.

    Conclusions:

    • Both ZMA00 and FLEX IOLs provide excellent distance and intermediate visual acuity for age-related cataract patients.
    • The ZMA00 multifocal IOL offers superior near visual outcomes compared to the FLEX accommodative IOL.
