Background:

Primary renal vein leiomyosarcoma (LMS) is an exceptionally rare vascular tumor.
Diagnosis is often delayed due to nonspecific clinical and radiological findings.

Observation:

This report details a case of a 50-year-old female diagnosed with LMS of the right renal vein.
The patient presented with symptoms that were not specific to the tumor's location or type.

Findings:

Leiomyosarcoma of the renal vein presents diagnostic challenges.
Tumor size greater than 3 cm is associated with increased risk of local recurrence and distant metastasis.
The overall prognosis for renal vein LMS is considered poor.
