Summary

Cavernous sinus cavernous hemangiomas (CSCH) can be effectively resected using a completely extradural transcavernous approach. This method achieves gross total excision with acceptable complications, offering a favorable treatment option for these rare skull base tumors.

Background:

  • Cavernous sinus cavernous hemangioma (CSCH) is a rare extra-axial vascular lesion.
  • CSCH presents surgical challenges due to location, bleeding risk, and proximity to neurovascular structures.

Purpose of the Study:

  • To report surgical experience with CSCH removal.
  • To evaluate the efficacy of a completely extradural transcavernous approach for CSCH resection.

Main Methods:

  • Retrospective study of 12 patients with CSCH.
  • Analysis of clinical symptoms, radiographic findings, surgical techniques, and outcomes.
  • Surgical removal via a purely extradural transcavernous approach.

Main Results:

  • Headache and visual impairment were common symptoms.
  • Characteristic CT and MRI findings were observed.
  • Gross total excision was achieved in all 12 patients.
  • Transient cranial nerve dysfunction occurred in 8 patients; 3 had permanent VI nerve palsy.
  • No recurrence was noted during a mean follow-up of 62 months.

Conclusions:

  • CSCHs are rare and challenging skull base tumors.
  • Microsurgical resection via an extradural transcavernous approach allows complete tumor resection.
  • This approach offers acceptable intraoperative and postoperative complications and should be considered a favorable treatment option.
