Background:

  • Meckel's diverticulum (MD) is definitively treated by surgical extirpation.
  • Conventional or laparoscopic approaches are standard, but challenging cases require innovative methods.

Observation:

  • A patient with suspected MD underwent a negative exploratory laparotomy.
  • Diagnosis was confirmed by gastrointestinal bleeding and isotopic tests, despite negative initial surgery.
  • A previously unidentified diverticulum was located 60-70 cm from the ileocecal valve.

Findings:

  • Laparoscopic radioguided surgery, utilizing a gamma probe, precisely located and resected the MD.
  • An intracorporeal termino-lateral anastomosis was successfully performed.
  • The patient experienced an uneventful recovery and was discharged on postoperative day five.

Implications:

  • Radioguided surgery combined with SPECT/CT offers a viable solution for surgically challenging MD cases.
  • This approach enhances precision in locating and resecting lesions in difficult abdominal areas.
  • It represents an advancement for patients with complex or previously missed gastrointestinal pathologies.
