P16 methylation in oral epithelial dysplasia (OED) predicts oral cancer development. This biomarker shows significant potential for early cancer detection, improving patient outcomes.
Area of Science:
- Oncology
- Molecular Biology
- Cancer Genetics
Background:
- P16 gene silencing via methylation or deletion is a frequent early event in cancer.
- Oral epithelial dysplasia (OED) is a precancerous condition with varying malignant potential.
Purpose of the Study:
- To prospectively evaluate P16 methylation as an early predictor of oral cancer development in patients with OED.
- To assess the diagnostic accuracy of P16 methylation for identifying malignant potential in OED.
Main Methods:
- A prospective, double-blind, multicenter cohort study involving 181 patients with mild or moderate OED.
- P16 methylation analysis using the MethyLight assay.
- Minimum 3-year follow-up to monitor OED progression and cancer development.
Main Results:
- Of 152 informative cases, 21 patients (14.3%) developed OED-derived squamous cell carcinoma during follow-up (median 41 months).
- P16 methylation-positive patients had a significantly higher cancer progression rate (27.1%) compared to methylation-negative patients (8.1%; adjusted OR=4.6, P=0.006).
- P16 methylation demonstrated a sensitivity of 62% and specificity of 76% for predicting cancer development from OED.
Conclusions:
- P16 methylation is a reliable biomarker for assessing the malignant potential of oral epithelial dysplasia.
- This finding supports the use of P16 methylation testing for early oral cancer risk stratification.
