Summary

Laparoscopic left colectomy (LC) has more postoperative complications and longer operative times than laparoscopic right colectomy (RC). These findings suggest LC and RC should be analyzed as distinct surgical procedures.

Background:

  • Laparoscopic colectomy for left-sided (LC) versus right-sided (RC) conditions is debated regarding technical difficulty and complication rates.
  • Existing literature lacks consensus on short-term outcome differences between laparoscopic right colectomy and laparoscopic left colectomy.

Purpose of the Study:

  • To compare the postoperative course of patients undergoing laparoscopic right colectomy versus laparoscopic left colectomy.
  • To evaluate differences in intraoperative and postoperative outcomes between LC and RC.

Main Methods:

  • Retrospective analysis of 1000 consecutive patients undergoing laparoscopic colectomy (RC or LC) from 1998 to 2012.
  • Analysis of intraoperative complications, operative time, postoperative complications, and length of stay.
  • Stratification into subgroups based on indication: neoplasia, diverticular disease, polyps, and others.

Main Results:

  • Laparoscopic left colectomy was associated with higher postoperative complication rates (30% vs. 19%) and longer operative times (139 min vs. 118 min) compared to laparoscopic right colectomy.
  • No significant differences were observed in reintervention or mortality rates between the groups.
  • Within laparoscopic left colectomy, diverticular disease cases had longer operative times (155 min vs. 137 min) and higher conversion rates (21% vs. 8%) compared to neoplasia cases.

Conclusions:

  • Laparoscopic left colectomy presents a higher risk of postoperative complications than laparoscopic right colectomy in a large patient cohort.
  • These results highlight distinct intraoperative and postoperative profiles for LC and RC.
  • The findings support treating right and left colectomies as separate surgical entities due to their differing outcomes.
