[Morphologic and morphometric characteristics of hypertrophic cardiomyopathy]
Insights
This study identifies key histological features in endomyocardial biopsies for diagnosing hypertrophic cardiomyopathy (HCMP). A scoring system based on these features demonstrated high sensitivity and specificity for HCMP diagnosis.
Area of Science:
- Cardiology
- Pathology
- Histology
Background:
- Hypertrophic cardiomyopathy (HCMP) diagnosis relies on clinical and imaging findings.
- Histological examination of endomyocardial biopsies offers detailed cellular insights.
Purpose of the Study:
- To identify and quantify characteristic histological features of HCMP in endomyocardial biopsies.
- To develop and validate a histological scoring system for HCMP diagnosis and differential diagnosis.
Main Methods:
- Analysis of 70 endomyocardial biopsies from 23 patients with hypertrophic cardiomyopathy, dilated cardiomyopathy, and chronic myocarditis.
- Evaluation of specific histological parameters: muscle fiber hypertrophy, cardiomyocyte nuclear hypertrophy, perinuclear halo, fiber disorganization, and nucleus-cytoplasmic ratio.
- Development of a scoring system (0-3 for each parameter) with a maximum total score of 15.
Main Results:
- Characteristic HCMP features identified include muscle fiber and cardiomyocyte nuclear hypertrophy, perinuclear halo, fiber disorganization, and decreased nucleus-cytoplasmic ratio.
- A histological index >60% confirmed HCMP diagnosis.
- The developed method showed high sensitivity (87.5%) and specificity (95%) in diagnosing HCMP.
Conclusions:
- A validated histological scoring system based on specific endomyocardial biopsy features aids in the diagnosis and differential diagnosis of hypertrophic cardiomyopathy.
- This method provides a sensitive and specific tool for confirming HCMP, distinguishing it from other cardiomyopathies.
Abstract:
Seventy endomyocardial biopsies of the left and right ventricular wall taken from 23 patients during catheterization of the heart cavities have been studied. There were three groups of patients: those with hypertrophic cardiomyopathy (HCMP) (7 patients), those with dilated cardiomyopathy (11) and 5 patients with chronic myocarditis. The most characteristic features of HCMP were as follows: 1) muscle fiber hypertrophy, 2) hypertrophy of cardiomyocyte nuclei, 3) perinuclear cardiomyocyte nimbus, 4) disorganization of muscle fibers, 5) the decrease of the nucleus-cytoplasmic ration. The degree of these parameters was scored 0 to 3. Maximum total score was 15 (100%). If histological index of HCMP was higher that 60%, the diagnosis of HCMP was considered to be confirmed. The method possesses a high sensitivity (87.5%) and specificity (95%) and can be used in the diagnosis and differential diagnosis of HCMP.
