Summary

This study on endometrial cancer found pelvic lymph node metastases in 15.1% and paraaortic lymph node metastases in 8.5% of patients. Primary surgical staging with comprehensive lymphadenectomy is recommended for all clinical Stage 1 cases.

Area of Science:

  • Gynecologic Oncology
  • Surgical Oncology
  • Pathology

Background:

  • Endometrial cancer is a common gynecologic malignancy.
  • Accurate staging is crucial for effective treatment planning.
  • The role of lymph node dissection in early-stage disease requires clarification.

Purpose of the Study:

  • To determine the incidence of pelvic and paraaortic lymph node metastases in clinical Stage 1 endometrial cancer.
  • To evaluate prognostic factors in relation to nodal status.
  • To support the utility of comprehensive lymphadenectomy in primary surgical staging.

Main Methods:

  • Retrospective analysis of 106 patients with clinical Stage 1 endometrial cancer.
  • Primary surgical staging including peritoneal cytology, type II radical hysterectomy, bilateral salpingo-oophorectomy.

Related Experiment Videos

  • Pelvic and paraaortic lymphadenectomy performed on all patients.
  • Main Results:

    • Pelvic lymph node metastases detected in 15.1% of patients.
    • Paraaortic lymph node metastases detected in 8.5% of patients.
    • Analysis of prognostic factors did not alter the recommendation for comprehensive lymphadenectomy.

    Conclusions:

    • Comprehensive pelvic and paraaortic lymphadenectomy is valuable for primary surgical staging in Stage 1 endometrial cancer.
    • The findings support routine lymphadenectomy regardless of identified risk factors.
    • This approach enhances accurate staging and treatment stratification.
