Summary

Dense nuclear cataracts and peripheral corneal degenerations increase the risk of transient corneal edema after phacoemulsification. Using the phaco-chop technique and minimizing phaco-power time can reduce this complication.

Background:

  • Transient corneal edema is a potential complication following phacoemulsification.
  • Identifying pre-operative and intra-operative risk factors is crucial for patient management.

Purpose of the Study:

  • To investigate factors associated with transient corneal edema after phacoemulsification and Intraocular Lens (IOL) implantation.

Main Methods:

  • A cohort study was conducted with 182 patients undergoing phacoemulsification.
  • Pre-operative factors (cataract density, corneal degenerations) and surgical factors (technique, phaco-power time) were analyzed.
  • Postoperative corneal clarity and striate keratopathy were assessed.

Main Results:

  • Hypertension, dense nuclear cataracts, divide and conquer technique, prolonged phaco-power use, and peripheral corneal degenerations were associated with increased risk.
  • Peripheral corneal degenerations (p < 0.001) and dense nuclear cataracts (p=0.006) showed significant association.

Conclusions:

  • Peripheral corneal degenerations and dense nuclear cataracts are significant risk factors for postoperative corneal edema.
  • Employing the phaco-chop technique and reducing phaco-power time can mitigate the incidence of corneal edema.
