Summary

Chylous ascites is a rare complication after laparoscopic nephrectomy. Most cases can be managed conservatively with dietary changes, total parenteral nutrition, or octreotide, avoiding surgery.

Background:

Chylous ascites is a rare but serious complication following laparoscopic nephrectomy.
It can lead to significant morbidity if not managed promptly.

Purpose of the Study:

To develop a management protocol for chylous ascites post-laparoscopic nephrectomy.
To outline strategies for prevention and treatment based on clinical experience.

Main Methods:

Retrospective review of patients who developed chylous ascites after laparoscopic nephrectomy (Jan 2010 - Jan 2014).
Analysis of the management protocols employed for these patients.

Main Results:

The incidence of chylous ascites was 0.77%, more frequent after left-sided and simple nephrectomies.
Conservative management (dietary modification, TPN, octreotide) was successful in 6 out of 9 patients.
Surgical intervention was required in 3 patients.

Conclusions:

Chylous ascites following laparoscopic nephrectomy is uncommon but associated with significant morbidity.
A proposed protocol emphasizes symptom-based and drainage-volume-guided management, including conservative measures and surgical options.
