Area of Science:

  • Medical Imaging
  • Fetal Ultrasound
  • Obstetrics

Background:

  • Nuchal translucency (NT) measurement is a critical first-trimester screening tool for fetal aneuploidies.
  • Accurate NT measurement is essential for reliable risk assessment.
  • Volume NT software offers advanced 3D analysis but requires optimization for consistent results.

Purpose of the Study:

  • To assess the impact of mid-sagittal plane deviation, fetal image size, tissue harmonic imaging (THI), and speckle reduction filter (SRF) on NT thickness measurements.
  • To determine the optimal settings within Volume NT software for accurate and reproducible NT measurements.
  • To compare NT measurements obtained with Volume NT software against traditional 2D ultrasound.

Main Methods:

  • Seventy-nine pregnant women underwent NT measurements using Volume NT software.
  • Three-dimensional volumes were analyzed based on angulation from the mid-sagittal plane (10° intervals).
  • Fetal image size was manipulated (downsizing, magnifying), and NT was measured with THI and SRF functions variously enabled or disabled.

Main Results:

  • Significant mean differences in NT measurements were observed with deviations of 31-40° (-0.09 mm) and 41-50° (-0.10 mm) from the mid-sagittal plane compared to 2D.
  • Intraclass correlation coefficients (ICC) for NT measurements improved with image magnification, reaching 0.928 for Method C.
  • The highest ICC (0.786) between 2D-NT and Volume NT measurements was achieved when both THI and SRF functions were utilized.

Conclusions:

  • NT measurements using Volume NT software are demonstrably affected by deviations from the mid-sagittal plane and variations in fetal image size.
  • The combined use of tissue harmonic imaging (THI) and speckle reduction filter (SRF) functions yields the highest correlation with traditional 2D NT measurements.
  • Optimizing image acquisition parameters, including angle and size, is crucial for accurate NT assessment with Volume NT software.
