Area of Science:

  • Orthopedic Surgery
  • Infectious Disease Management

Background:

  • Shoulder joint empyema can lead to complex post-operative complications.
  • Recurrent subcutaneous abscesses pose significant treatment challenges.

Observation:

  • A patient presented with extensive subcutaneous abscesses following shoulder joint empyema.
  • The patient underwent twelve surgical interventions over a 14-month period.

Findings:

  • The persistent abscesses were ultimately linked to an undiagnosed epidermoid cyst.
  • Complete extirpation of the epidermoid cyst resulted in the cessation of abscess formation.

Implications:

  • Highlights the importance of thorough differential diagnosis in persistent post-surgical infections.
  • Demonstrates the efficacy of complete cyst removal in resolving complex abscesses.
  • Informs surgical strategies for managing challenging shoulder joint infections.
