Summary

Serum miR-99a shows promise as a biomarker for acute rejection (AR) in renal transplant patients. This microRNA can help distinguish AR from stable transplant conditions and delayed graft function.

Area of Science:

  • Biochemistry
  • Molecular Biology
  • Transplantation Medicine

Background:

  • MicroRNAs (miRNAs) are non-coding RNAs with potential as disease biomarkers.
  • Circulating miRNAs in renal transplant patients require further investigation.

Purpose of the Study:

  • To identify circulating microRNAs as potential biomarkers for acute rejection (AR) in renal transplantation.
  • To evaluate the diagnostic capacity of serum miRNAs in distinguishing AR and delayed graft function (DGF).

Main Methods:

  • Serum miRNA profiling using an miRNA array in stable and AR patients.
  • Validation of differentially expressed miRNAs via quantitative real-time polymerase chain reaction in AR, control, and DGF patient cohorts.
  • Receiver operator characteristic (ROC) analysis to assess diagnostic accuracy.

Main Results:

  • Several miRNAs (miR-99a, miR-100, miR-151a, let-7a, let-7c, let-7f) were deregulated in AR patients.
  • miR-99a and miR-100 were significantly upregulated in the AR group upon validation.
  • Serum miR-99a demonstrated significant diagnostic value in differentiating AR from stable patients (AUC=0.750) and DGF patients (AUC=0.811).

Conclusions:

  • Serum miR-99a is a potential non-invasive biomarker for detecting acute rejection in renal transplant recipients.
  • Further research is warranted to validate these findings and explore clinical utility.
