Opsoclonus-myoclonus syndrome associated with multiple system atrophy

Opsoclonus-myoclonus syndrome (OMS) is rarely associated with neurodegenerative disorders. This case report details a patient with advanced multiple system atrophy-parkinsonian type (MSA-P) who developed OMS, a previously undescribed association.

Background:

- A 48-year-old woman with a 5-year history of multiple system atrophy-parkinsonian type (MSA-P) presented with new-onset involuntary movements.
- The patient exhibited rapid, random, multidirectional conjugate saccades (opsoclonus) and asynchronous, arrhythmic myoclonic jerks.
Findings:
- The patient was diagnosed with advanced MSA-P associated with opsoclonus-myoclonus syndrome.
- This represents the first reported instance of OMS occurring in the context of MSA-P.
Implications:
- This finding expands the known clinical spectrum of OMS.
- It suggests a potential, previously unrecognized link between neurodegenerative processes and autoimmune or inflammatory responses manifesting as OMS.
