Abstract:
I am grateful to Andrew Leaver in his role as chairman of the NSW Chapter of Musculoskeletal Physiotherapy Australia for raising issues pertinent to our paper. It is this interest that encourages debate and helps to evolve further the ideas on a topic that has not been investigated previously.
