Upper lumbar disc herniations are uncommon but present distinct symptoms like anterior thigh pain, unlike typical sciatica. This study found a high rate of cauda equina syndrome and excellent surgical outcomes in these cases.

    Area of Science:

    • Neurosurgery
    • Orthopedic Surgery
    • Spinal Disorders

    Background:

    • Upper lumbar disc herniations are infrequently diagnosed.
    • A retrospective review of 457 lumbar disc surgeries identified 22 cases of upper lumbar disc herniation.

    Purpose of the Study:

    • To analyze the clinical syndromes, radiological findings, and surgical outcomes of upper lumbar disc herniations.
    • To differentiate the presentation of upper lumbar disc herniation from lower lumbar disc herniation.

    Main Methods:

    • Retrospective case series analysis.
    • Review of clinical data, radiological findings, and surgical outcomes for 22 patients.
    • Comparison of clinical features with lower lumbar disc herniations.

    Main Results:

    Related Experiment Videos

    • Upper lumbar disc herniation presented with anterior thigh pain, distinct from the sciatica of lower lumbar herniations.
    • Cauda equina syndrome occurred in 36% of cases.
    • 90% of patients (20/22) achieved satisfactory surgical results.

    Conclusions:

    • Upper lumbar disc herniation has unique clinical features and a high incidence of cauda equina syndrome.
    • Surgical intervention for upper lumbar disc herniation yields favorable outcomes.
