Multiple sclerosis (MS) myelitis often presents in younger patients with multifocal spinal cord lesions. Differentiating MS myelitis from other causes is crucial for accurate diagnosis and treatment.
Area of Science:
- Neurology
- Immunology
- Radiology
Background:
- Myelitis can be an initial symptom of various conditions, including demyelinating, inflammatory, and infectious diseases.
- Distinguishing myelitis associated with multiple sclerosis (MS) from other causes is clinically significant.
Purpose of the Study:
- To analyze the differences between myelitis associated with multiple sclerosis (MS) and myelitis resulting from other etiologies.
- To identify clinical and radiological features that differentiate MS-related myelitis.
Main Methods:
- Retrospective analysis of 91 patients with initial myelitis from 2000-2013 at a single center.
- Comparison of demographic, etiological, clinical, radiological, and prognostic variables between MS and non-MS myelitis groups.
Main Results:
- MS-related myelitis occurred in younger patients, showed more sphincter and multifocal spinal MRI involvement, shorter lesions, and posterior/cervical locations.
- Other etiologies more frequently presented with anterior/central cord involvement and had better recovery at one year.
- Multivariate analysis identified multifocal and posterior spinal cord involvement as independently associated with MS diagnosis.
Conclusions:
- A significant proportion of initial myelitis cases are diagnosed as MS.
- Multifocal and posterior spinal cord lesions are key indicators associated with MS diagnosis in patients presenting with myelitis.
