Miliary sarcoidosis, a rare lung condition, can cause shortness of breath. Early diagnosis and glucocorticoid treatment effectively resolved symptoms and miliary opacities in a patient.

- Consider sarcoidosis alongside tuberculosis, malignancy, and pneumoconiosis in patients with miliary opacities.
