Insights
Vitamin D supplementation improved cardiac and endothelial function in children with chronic kidney disease (CKD). This study highlights the importance of vitamin D for cardiovascular health in pediatric CKD patients.
Area of Science:
- Pediatric Nephrology
- Cardiovascular Health
- Endocrinology
Background:
- Cardiovascular disease is a primary cause of mortality in children with chronic kidney disease (CKD).
- Vitamin D deficiency is common in pediatric CKD patients, potentially exacerbating cardiovascular risks.
Purpose of the Study:
- To investigate the impact of oral cholecalciferol (vitamin D3) supplementation on cardiac parameters.
- To assess changes in biomarkers of endothelial cell activation in children with CKD following vitamin D treatment.
Main Methods:
- Echocardiography and vascular stiffness measurements were performed basally and post-supplementation in 41 children with CKD and 24 healthy controls.
- Evaluated markers included flow-mediated dilatation (FMD), arterial stiffness, homocysteine, and von Willebrand factor.
Main Results:
- Children with CKD exhibited lower FMD and higher arterial stiffness compared to controls.
- Vitamin D supplementation significantly improved FMD and reduced arterial stiffness in the CKD group.
- Homocysteine levels correlated inversely with baseline vitamin D, and von Willebrand factor was an independent risk factor for FMD impairment.
Conclusions:
- High-dose cholecalciferol positively impacts cardiovascular and endothelial function in children with CKD.
- Vitamin D supplementation is crucial for managing cardiovascular risk in pediatric CKD.
Background:
As cardiovascular factors are the leading cause of mortality in chronic kidney disease (CKD) and as vitamin D deficiency is prevalent in this population, we aimed to examine the effect of oral cholecalciferol on cardiac parameters and biomarkers for endothelial cell activation in children with CKD.
Methods:
Forty-one children with CKD and 24 healthy subjects free of any underlying cardiac or renal disease with low 25-hydroxyvitamin D3 (25OHD) levels were evaluated using echocardiography basally and following Stoss vitamin D supplementation. The local vascular stiffness and endothelial dysfunction markers were compared among the groups.
Results:
Initial flow-mediated dilatation (FMD) measurements were lower and local arterial stiffness was significantly higher in patients. After vitamin D supplementation, these improved significantly in patients, while no significant change was observed for the healthy group. Homocysteine showed inverse correlation with baseline vitamin D level in CKD children and von Willebrand factor emerged as an independent risk factor for FMD impairment.
Conclusions:
Our interventional study revealed the favorable effects of high-dose cholecalciferol on cardiovascular and endothelial parameters, implying the importance of vitamin D supplementation in children with CKD.

Role of Vitamins in Maintaining Bone Health
Vitamin A
Vitamin A is involved in the process of bone remodeling. Retinoic acid, the active metabolite of Vitamin A, has nuclear receptors in osteoblasts and osteoclasts, which are involved in bone remodeling.
Vitamin B12
Vitamin B12 acts as a cofactor during the formation of osteoblast-related proteins, such as osteocalcin. Vitamin B12 plays a role...
Renal Failure: Dose Adjustments
Reduced renal clearance and elimination rate are common outcomes of renal impairment. These alterations lead to a prolonged elimination half-life and an altered apparent volume of distribution for drugs. As a result, dosage adjustments are typically necessary to maintain optimal drug levels in the body.
However, dosage adjustments...
