Summary

Orthodontic bracket primer use showed similar overall failure rates, but significantly reduced failures in younger patients. This suggests primer is beneficial for younger children undergoing orthodontic treatment.

Purpose of the Study:

  • To compare the incidence of bracket failure in orthodontic treatment when using primer versus not using primer.
  • To evaluate the efficacy of Transbond™ MIP primer in conjunction with Transbond™ XT adhesive for bracket bonding.

Main Methods:

  • A single-operator, cross-mouth, randomized controlled trial (RCT) was conducted.
  • Fifty patients undergoing bimaxillary orthodontic treatment were included.
  • Bracket failures were recorded over an 18-month observation period, with quadrants randomly assigned to primer or non-primer groups.

Main Results:

  • Overall bracket failure rates were 5.5% without primer and 3.1% with primer (P=0.063).
  • Younger patients (10-13 years) showed significantly higher failure rates without primer (12.1%) compared to with primer (4.1%) (P<0.001).
  • No significant difference in failure rates was observed between primer and non-primer groups for older patients (14-18 years).

Conclusions:

  • Bonding orthodontic brackets with or without primer demonstrated similar overall clinical effectiveness.
  • The use of primer significantly reduced bracket failure rates in younger orthodontic patients.
  • Primer application may be particularly beneficial for improving bond durability in pediatric patients.
