Summary

This study shows that the Master and Slave Transluminal Endoscopic Robot (MASTER) enabled novices to perform endoscopic submucosal dissection (ESD). While expert endoscopists trended towards shorter operative times, the robotic system facilitated procedure completion for all skill levels.

Background:

  • Endoscopic submucosal dissection (ESD) presents challenges due to a lack of counter traction.
  • The Master and Slave Transluminal Endoscopic Robot (MASTER) was developed to enhance control during complex endoluminal procedures.

Purpose of the Study:

  • To compare the performance of ESD between experienced endoscopists and novices using the MASTER robotic system.
  • To evaluate the feasibility of robotic-assisted ESD for individuals with varying levels of clinical experience.

Main Methods:

  • A prospective study was conducted using an ex vivo porcine stomach model.
  • Participants with and without surgical/endoscopic experience performed standardized ESD procedures using MASTER.
  • Operative time and specimen size were recorded after basic robotic control training.

Main Results:

  • Nine participants (3 expert endoscopists, 3 non-expert endoscopists, 3 novices) completed the ESD procedures en bloc without perforation.
  • Mean operative times were 122s (experts), 203s (non-experts), and 561s (novices).
  • No statistically significant difference in operative time was found between the groups (P=0.242), though endoscopists trended faster (162s vs. 561s, P=0.085).

Conclusions:

  • The MASTER system facilitated ESD completion for novices lacking prior endoscopy experience.
  • A trend towards shorter operative times was observed for endoscopists compared to non-endoscopists using the robotic system.
