    Summary

    Endometrial stromal sarcoma is a rare cancer. This case study details a patient with recurrent endometrial stromal sarcoma treated with total pelvic exenteration.

    Background:

    • Endometrial stromal sarcomas (ESS) are rare uterine malignancies originating from endometrial stromal cells.
    • ESS are categorized into low-grade and high-grade subtypes based on mitotic index, with high-grade ESS associated with poorer prognosis and higher recurrence rates.

    Observation:

    • A 46-year-old female patient with a history of surgically treated endometrial stromal sarcoma and adjuvant radiotherapy presented with pelvic recurrence.
    • The recurrence involved invasion of the urinary bladder trigone and rectosigmoid.

    Findings:

    • The patient underwent a total pelvic exenteration for the recurrent endometrial stromal sarcoma.
    • This radical surgical approach addresses extensive pelvic recurrences of ESS.
