Summary

Tumor seeding along the percutaneous transhepatic biliary decompression tract is a rare but serious complication. Routine preoperative biliary drainage should be avoided in patients with potentially resectable biliary tract cancers.

Background:

  • Percutaneous transhepatic biliary decompression (PTBD) is a common preoperative adjunct for obstructive jaundice.
  • Tumor seeding along the PTBD catheter tract is an uncommon complication.

Observation:

  • A patient developed a metastatic adenocarcinoma nodule at the PTBD catheter exit site four months after pancreaticoduodenectomy.
  • Autopsy revealed extensive tumor seeding along the catheter tract.

Findings:

  • A literature review identified 17 additional cases of PTBD catheter tract seeding.
  • Most cases involved palliation (13/18), but seeding occurred even after short-term drainage before curative resections (4/5).
  • Cholangiocarcinoma and pancreatic carcinoma were the most common primary tumors (9/18 and 7/18, respectively).

  • Catheter tip position and duration of drainage did not prevent tumor seeding.
  • Routine preoperative PTBD is not recommended for potentially resectable malignant biliary obstruction.
  • Palliative bypass may be considered if tumors are unresectable upon exploration.
