Abstract:
The rising number of cases of people with dementia, the need for complex care and the insufficient care available call for innovative and sustainable solutions. The article presents a specialised dementia care model that has been developed to manage acute difficulties of care. The model implements comprehensive, patient-oriented care across different sectors. The structure and requirements of successful cooperation and comprehensive care will be described.
