Background:

Purpose of the Study:

  • To investigate the presence and activity of tissue factor (TF) in a sheep model of prosthetic dialysis grafts.
  • To understand the role of TF in the development of intimal hyperplasia and graft failure.

Main Methods:

  • Expanded polytetrafluoroethylene (ePTFE) grafts were implanted in sheep.
  • Luminal TF activity was measured using an ex vivo assay.
  • Immunohistochemistry localized TF protein, smooth muscle cells, and monocytes.

Main Results:

  • Elevated luminal TF activity was observed as early as 2 days post-engraftment, persisting for 8 weeks.
  • TF activity was significantly higher in the venous end of the grafts compared to the arterial end.
  • TF protein colocalized with fibrin accumulation near the lumen, suggesting a role in thrombus organization.

Conclusions:

  • TF levels are increased on the luminal surface of ePTFE grafts and adjacent vessels, correlating with intimal hyperplasia.
  • Increased TF activity is linked to luminal thrombus formation, providing a scaffold for hyperplasia.
  • Targeting luminal TF activity presents a potential therapeutic strategy to prevent graft failure.
