Summary

Mohs micrographic surgery (MMS) offers a functional alternative to amputation for digital melanoma, with low recurrence rates. Outcomes are further improved by using Melanocytic antigen recognized by cytotoxic T lymphocytes from melanoma patients (MART)-1.

Background:

  • Digital melanoma is typically treated with amputation or wide local excision.

Purpose of the Study:

  • To evaluate the outcomes of digital melanomas treated with MMS over a 35-year period.
  • To assess the efficacy of MMS in preserving function and minimizing recurrence in digital melanoma cases.

Main Methods:

  • A retrospective analysis of digital melanomas treated with MMS was conducted.
  • Tumor characteristics, treatment details, and patient follow-up data were collected and analyzed.
  • Immunohistochemical stains, including Melanocytic antigen recognized by cytotoxic T lymphocytes from melanoma patients (MART)-1 and HMB-45, were utilized.

Main Results:

  • Out of 4995 melanomas, 62 (1.2%) were digital; 57 were primary and 5 were recurrent.
  • Local recurrence occurred in 5 (8.2%) tumors, none with MART-1 use. Three recurrences were successfully managed with additional MMS.
  • Five-year local recurrence-free survival was 91.8% for primary melanomas. 96.5% of patients avoided amputation, and 5-year melanoma-specific survival was 95.0%.

Conclusions:

  • MMS is an effective treatment for digital melanoma, conserving function by avoiding amputation and achieving a low local recurrence rate.
  • The use of MART-1 immunostains appears to improve outcomes.
  • MMS offers a viable, function-sparing surgical option for digital melanoma management.
