Summary

Higher levels of soluble CD14 (sCD14), an inflammatory marker, are linked to an increased risk of hip fractures in white adults. This finding suggests sCD14 may help identify individuals at higher risk for fractures.

Background:

  • Soluble CD14 (sCD14) is a proinflammatory cytokine originating from macrophages/monocytes.
  • These cells can differentiate into osteoclasts, which are involved in bone resorption.
  • The study investigated the link between sCD14 levels and osteoporotic fractures.

Purpose of the Study:

  • To examine the relationship between soluble CD14 (sCD14) levels and the risk of osteoporotic fractures.
  • To determine if sCD14 is a potential biomarker for fracture risk assessment.

Main Methods:

  • The Cardiovascular Health Study cohort included 5462 participants with baseline sCD14 measurements.
  • Incident hip fractures and composite fractures (hip, pelvis, humerus, distal radius) were tracked over a median of 12.5 and 8.6 years, respectively.
  • Cox proportional hazards models analyzed the association between sCD14 levels and fracture risk, stratified by race and gender.

Main Results:

  • Unadjusted models showed a positive association between higher sCD14 levels and increased risk of hip and composite fractures.
  • After adjusting for demographics, lifestyle, and medications, this association was no longer significant overall.
  • However, in white participants, higher sCD14 levels remained significantly associated with an increased risk of hip fractures in fully adjusted models.

Conclusions:

  • Elevated sCD14 levels are associated with an increased risk of incident hip fractures specifically in white older adults.
  • sCD14 may serve as a valuable marker for identifying fracture risk in certain populations.
