Background:

Disability in multiple sclerosis (MS) significantly impacts patient quality of life, caregiver burden, and healthcare utilization.
Current healthcare models inadequately address the complex needs of progressive MS patients, leading to preventable complications and distress.
A novel home-based comprehensive care model is proposed to mitigate these challenges.

Purpose of the Study:

To describe the study design and methods for implementing and evaluating a home-based comprehensive care program for progressive MS.
To assess the feasibility and effectiveness of the proposed intervention over 24 months.

Main Methods:

A 24-month, randomized controlled trial evaluating a home-based, patient-centered comprehensive care program.
A transdisciplinary team led by an MS-Comprehensivist will manage the intervention.
Fifty MS patients will be randomized to either the intervention group or the usual care group.

Main Results:

Hypothesized outcomes include fewer complications, improved quality of life, higher patient satisfaction, and reduced healthcare utilization in the intervention group.
The study will assess the feasibility and effectiveness of the home-based model.
Financial sustainability in various healthcare payment models will be evaluated.

Conclusions:

This is the first study to examine a home-based comprehensive care management program for progressive MS patients with disabilities.
Successful implementation could significantly improve care delivery, research, education, and practice for complex chronic conditions.
The model aims to provide high-quality, affordable care for individuals with severe, disabling neurological conditions.
