Background:

  • Systemic sclerosis is a chronic autoimmune disease characterized by fibrosis and vascular abnormalities.
  • Cosmetic augmentation mammoplasty using silicone-gel implants is a common procedure.

Observation:

  • Four women developed systemic sclerosis 6-15 years post-mammoplasty.
  • Patients exhibited Raynaud phenomenon, arthralgia, and organ involvement.
  • Enlarged lymph nodes near prostheses were observed in two patients.

Findings:

  • Silicone leakage from implants was evidenced by refractile particles and silicon identification in distant tissues.
  • Chronic inflammation with lymphocytes, histiocytes, and giant cells was present.
  • Removal of implants did not improve systemic sclerosis in two cases.

Implications:

  • Silicone leakage from breast implants may contribute to systemic sclerosis development.
  • Further research is needed to elucidate the mechanism of silicone-induced autoimmunity.
  • This study highlights a potential environmental trigger for autoimmune diseases.
