    • Data from 357 patients receiving isotretinoin between 2006 and 2010 were analyzed.
    • Congruence and discrepancies between medical documentation and iPLEDGE records were evaluated.

    Main Results:

    • Only 73.1% of cases showed complete agreement between medical charts and iPLEDGE data.
    • Discrepancies occurred in 96 cases, with 81.4% attributed to missed doses in the iPLEDGE system.
    • A total of 249 discrepancies were identified, including missed doses and unrecorded dispensations.

    Conclusions:

    • Significant data discordance was observed between iPLEDGE and patient medical charts.
    • The findings challenge the efficacy of the iPLEDGE system in accurately tracking every isotretinoin dose.
    • Improvements in data management within iPLEDGE are warranted to ensure accurate patient monitoring.
