Background:

  • Rett syndrome is a neurodevelopmental disorder caused by MECP2 gene mutations.
  • Impaired gross motor function is a key characteristic of Rett syndrome.
  • The Rett Syndrome Gross Motor Scale (RSGMS) was developed to assess motor abilities.

Purpose of the Study:

  • To investigate the validity and reliability of the Rett Syndrome Gross Motor Scale.
  • To analyze the factor structure of the RSGMS and its relationship with age and genotype.
  • To assess the repeatability of clinical assessments using the RSGMS.

Main Methods:

  • Collected video and parent report data from 255 individuals with Rett syndrome.
  • Utilized principal components analysis to determine factor scores (Sitting, Standing and Walking, Challenge).
  • Assessed measurement consistency using clinical assessment scores from 38 individuals.

Main Results:

  • Identified three main factor scores: Sitting, Standing and Walking, and Challenge.
  • Observed poorer motor scores with increasing age.
  • Found that specific MECP2 mutations (p.Arg133Cys, p.Arg294*, p.Arg306Cys) correlated with higher scores compared to large deletions.
  • Demonstrated excellent repeatability of clinical assessment (ICC=0.99).

  • The Rett Syndrome Gross Motor Scale is a valid and reliable measure for assessing gross motor skills.
  • The scale is appropriate for use in clinical practice and clinical trials for Rett syndrome.
  • RSGMS findings correlate with age and specific MECP2 genotypes.
