    Summary

Postoperative stricture of the anal channel (SACH) is a common complication. New diagnostic and surgical methods significantly reduced SACH occurrence from 45.4% to 6.7%.

    Area of Science:

    • Proctology
    • Surgical Gastroenterology
    • Colorectal Surgery

    Background:

    • Postoperative stricture of the anal channel (SACH) is a significant complication following anorectal surgery.
    • Previous surgical techniques, including Milligan-Morgan hemorrhoidectomy, were associated with a high incidence of SACH (92% in this study).
    • Conventional treatment approaches led to a 45.4% occurrence rate of SACH in the comparison group.

    Purpose of the Study:

    • To analyze the treatment outcomes of 50 patients with SACH.
    • To evaluate the effectiveness of novel diagnostic and surgical strategies for managing SACH.
    • To compare the incidence of SACH before and after the implementation of improved techniques.

    Main Methods:

    • Retrospective analysis of 50 patients treated for SACH between 2006 and 2014.
    • Implementation of a new diagnostic approach using isolated roentgen contrast investigation of the anal channel.
    • Application of improved conservative and surgical treatment techniques for SACH in the main group (2008-2014).

    Main Results:

    • The incidence of SACH was significantly reduced from 45.4% in the conventional treatment group to 6.7% in the group treated with new approaches.
    • Roentgen contrast investigation objectively determined the form, diameter, and grade of anal channel strictures, aiding in surgical planning.
    • 39 patients (78%) in the main group underwent new diagnostic and treatment protocols, with 30 (76.9%) receiving surgical intervention.

    Conclusions:

    • The developed roentgen contrast investigation is a valuable screening tool for objectively assessing anal channel strictures.
    • Improved operative techniques for SACH have demonstrably lowered its occurrence rate.
    • The new diagnostic and surgical approaches represent a significant advancement in managing postoperative anal strictures.
