    Background:

    Purpose of the Study:

    • To evaluate the long-term effect of saxagliptin on renal function in type 2 diabetic patients.
    • To assess changes in albuminuria and estimated glomerular filtration rate (eGFR) as primary outcomes.
    • To explore secondary effects on clinical and laboratory parameters.

    Main Methods:

    • Pilot observational study in two diabetes clinics.
    • Inclusion criteria: type 2 diabetics on saxagliptin for ≥12 months without concurrent antihypertensive/lipid-lowering therapy changes.
    • Primary outcomes: albuminuria and eGFR; Secondary outcomes: clinical and laboratory parameters.

    Main Results:

    • After 12 months, mean albuminuria significantly decreased (39 to 22 mg/L, P<0.001).
    • Prevalence of increased albuminuria (>20 mg/L) reduced by 27%.
    • eGFR remained stable despite improved glycemic control (HbA1c from 7.1% to 6.7%).

    Conclusions:

    • Saxagliptin treatment in high-risk diabetic patients is associated with reduced albuminuria.
    • The drug appears to stabilize estimated glomerular filtration rate (eGFR) in this cohort.
    • Prospective trials are necessary to confirm saxagliptin's nephroprotective potential.
