    Background:

    Purpose of the Study:

    • To analyze the clinical and histological features of Plasmodium falciparum malaria patients presenting with jaundice.
    • To investigate the spectrum of complications and outcomes in this patient cohort.

    Main Methods:

    • Retrospective analysis of 32 smear-positive Plasmodium falciparum malaria patients with jaundice.
    • Review of clinical data, laboratory results (serum bilirubin, transaminases, azotaemia), and liver biopsy findings.

    Main Results:

    • Majority of patients were aged 31-40 years, with serum bilirubin levels frequently exceeding 10 mg%.
    • Conjugated hyperbilirubinemia was the predominant pattern (65.5%).
    • Associated complications included azotaemia (78%), intravascular hemolysis (34.3%), and possible cerebral malaria (9.3%). Hepatosplenomegaly was universal. Mortality was 21%.

    Conclusions:

    • Jaundice in Plasmodium falciparum malaria is associated with significant hepatic and renal dysfunction, hemolysis, and a considerable mortality rate.
    • Histological findings in the liver consistently showed reticulo-endothelial cell hyperplasia, Kupffer cell pigmentation, and other signs of inflammatory and cholestatic injury.
