Summary

Transient carotid artery compression during cardiac surgery significantly reduced solid cerebral emboli. This technique, monitored by transcranial Doppler (TCD), offers a promising method to decrease neurologic injury risk.

Purpose of the Study:

  • To investigate the impact of transient carotid artery compression on embolic load.
  • To assess the effect of this maneuver during cardiac surgery with cardiopulmonary bypass.
  • To determine if carotid compression reduces solid particle embolization.

Main Methods:

  • Prospective, randomized clinical trial.
  • 20 patients underwent carotid compression (Group I), 20 did not (Group II).
  • Bilateral middle cerebral artery monitoring using transcranial Doppler (TCD) to detect emboli during specific surgical maneuvers.

Conclusions:

  • Transient carotid artery compression during arterial cannulation and aortic declamping decreases solid cerebral emboli.
  • This preliminary trial suggests a potential strategy to mitigate neurologic injury risk.
  • Further randomized studies are needed to confirm efficacy in specific surgical contexts.
