Detecting secondary caries around dental restorations is crucial. Quantitative light-induced fluorescence (QLF) and Fluorescence Enamel Imaging (PCDS) show promise for composite resins, but methods are questionable for amalgam restorations.

Background:

  • Secondary caries is a primary cause for replacing dental restorations.
  • Advanced diagnostic tools can improve the accuracy of secondary caries detection.
  • This study assesses new methods for identifying secondary caries around amalgam and composite restorations.

Purpose of the Study:

  • To evaluate the performance of visual criteria (International Caries Detection and Assessment System - ICDAS) and optical devices for detecting secondary caries.
  • To compare these methods around amalgam and resin composite restorations in permanent teeth.

Main Methods:

  • 180 extracted teeth with Class I amalgam (n=90) and resin composite (n=90) restorations were analyzed.
  • Examiners used ICDAS visual criteria, laser fluorescence (LF), light-emitting diode (MID), quantitative light-induced fluorescence (QLF), and Fluorescence Enamel Imaging (PCDS).
  • Confocal laser scanning microscopy served as the gold standard for diagnosis.

Main Results:

  • All tested methods demonstrated high reproducibility, except for MID around amalgam restorations.
  • QLF and PCDS exhibited the highest sensitivity for detecting secondary caries in both groups.
  • Other methods showed superior specificity (p < 0.05).

Conclusions:

  • The MID device was not effective for secondary caries detection around either restoration type.
  • ICDAS, LF, QLF, and PCDS showed potential for detecting secondary caries around resin composite restorations.
  • The effectiveness of all evaluated methods for secondary caries detection around amalgam restorations remains questionable.
