Summary

The extensor carpi ulnaris (ECU) tendon frequently moves out of its groove, especially with the forearm supine. Shallower grooves are linked to increased ECU tendon translation in asymptomatic individuals.

Background:

  • The extensor carpi ulnaris (ECU) tendon is crucial for wrist stability.
  • Understanding its normal position within the ulnar groove is important for diagnosing wrist pathologies.

Purpose of the Study:

  • To assess ECU tendon position in the ulnar groove across different forearm orientations.
  • To determine the frequency of ECU tendon "dislocation" in asymptomatic volunteers.
  • To investigate the association between ulnar groove morphology and ECU tendon position.

Main Methods:

  • Axial proton density-weighted MRI was used to examine 38 asymptomatic wrists.
  • Forearm positions included prone, neutral, and supine.
  • Measurements included the percentage of tendon outside the groove, ulnar groove depth, and length.

  • 15.8% of tendons remained fully within the groove in all positions.
  • 76.3% of tendons showed medial translation from prone to supine.

Conclusions:

  • The ECU tendon commonly translates or dislocates from the ulnar groove, particularly in the supine position.
  • This phenomenon was observed in a majority of asymptomatic volunteers.
  • Ulnar groove morphology, specifically its depth and length, influences ECU tendon stability.
