  • This case focuses on a rare adverse reaction to atorvastatin.

Observation:

  • An 84-year-old male developed bilateral pleural effusions.
  • The effusions appeared two years after initiating atorvastatin therapy.
  • No alternative causes for the pleural effusion were identified.

Findings:

  • Discontinuation of atorvastatin led to rapid resolution of pleural effusions and associated symptoms.
  • Accidental re-challenge with atorvastatin resulted in the recurrence of pleural effusion.
  • These findings strongly suggest atorvastatin as the causative agent.

Implications:

  • Pleuropulmonary manifestations in patients on atorvastatin should prompt consideration of drug-induced etiology.
  • Discontinuation of atorvastatin should be a primary management strategy for suspected iatrogenic pleural effusion.
  • This case contributes to the understanding of rare adverse effects of statin therapy.
