New therapies are emerging to combat graft-versus-host disease (GVHD) after stem cell transplants. While promising, these novel treatments require further clinical trials to confirm efficacy against corticosteroids.
Area of Science:
- Hematology
- Immunology
- Oncology
Background:
- Graft-versus-host disease (GVHD) is a major cause of morbidity and mortality following allogeneic stem cell transplantation.
- Current corticosteroid treatments are often insufficient, leading to poor quality of life, increased infection risk, and mortality.
- There is a critical need for novel therapies to improve outcomes in acute and chronic GVHD.
Purpose of the Study:
- To review emerging drugs for the prevention and therapy of graft-versus-host disease (GVHD).
- To discuss the current pre-clinical and clinical data for novel GVHD therapeutic agents.
Main Methods:
- Review of emerging drugs for GVHD prevention and therapy.
- Discussion of cellular therapies.
- Analysis of pre-clinical and clinical data for monoclonal antibodies, interleukin-2, alpha-1 antitrypsin, histone deacetylase inhibitors, tyrosine kinase inhibitors, and proteasome inhibitors.
Main Results:
- Several novel therapeutic agents are in early-phase clinical trials for GVHD.
- Monoclonal antibodies, IL-2, alpha-1 antitrypsin, HDAC inhibitors, TKIs, and proteasome inhibitors are under investigation.
- Alpha-1 antitrypsin, ruxolitinib, and interleukin-2 have shown encouraging preliminary results.
Conclusions:
- Emerging therapies for GVHD show promise but require further randomized controlled trials.
- These novel agents are expected to improve response rates compared to corticosteroids alone.
- Alpha-1 antitrypsin, ruxolitinib, and IL-2 are particularly promising candidates for future GVHD treatment.
