Background:

  • Severe keratoconjunctivitis sicca (KCS) significantly impacts vision quality.
  • Increased straylight is a potential cause of visual discomfort in KCS patients.

Purpose of the Study:

  • To quantify vision quality complaints in KCS patients by measuring straylight.
  • To evaluate the effect of scleral lens wear on straylight in KCS patients.

Main Methods:

  • Straylight was measured using the Oculus C-Quant in 16 KCS patients.
  • Scleral lenses were worn by 9 patients, with straylight measured with and without lenses.
  • Corneal health was assessed via Schirmer test, visual acuity, van Bijsterveld score, and a straylight questionnaire.

Main Results:

  • KCS patients exhibited significantly higher straylight levels (log(s) = 1.36-1.45) compared to age-matched controls (log(s) = 1.10).
  • Scleral lens wear showed a non-significant trend towards reduced straylight (0.06 log units).
  • No correlation was found between straylight and the van Bijsterveld corneal damage score.

Conclusions:

  • Keratoconjunctivitis sicca significantly increases straylight, potentially affecting vision.
  • The source of increased straylight in KCS requires further investigation, as it's not directly correlated with surface damage.
  • Scleral lenses did not demonstrate a significant impact on straylight in this patient group.
