Summary

Nonconvulsive status epilepticus (NCSE) presents without major motor seizures. Early diagnosis using continuous EEG or neuroimaging is crucial for effective treatment of this condition.

Background:

  • Nonconvulsive status epilepticus (NCSE) is a subtype of status epilepticus characterized by a lack of prominent motor symptoms.
  • NCSE accounts for nearly half of all status epilepticus cases, with an estimated annual prevalence of 10-20 per 100,000 individuals.
  • Various forms of NCSE exist, including absence SE, complex partial SE, late-onset de novo NCSE, and NCSE secondary to acute brain injury or post-convulsive states.

Observation:

  • Diagnosis of NCSE necessitates electroencephalography (EEG).
  • Continuous EEG (cEEG) monitoring for at least 24 hours is recommended due to the fluctuating nature of cognitive disturbances in NCSE.
  • Neuroimaging, including MRI with arterial spin labeled sequences and SPECT, can reveal hyperperfusion in focal onset NCSE.

Findings:

  • NCSE is often underdiagnosed due to its subtle presentation.
