Summary

Phacoemulsification surgery can lead to posterior migration of the Malyugin ring, a complication causing visual disturbance. This iris expansion device can be safely removed from the vitreous using its original injector.

Area of Science:

  • Ophthalmology
  • Surgical Complications
  • Cataract Surgery

Background:

  • Phacoemulsification is a common cataract surgery technique.
  • Iris expansion devices, like the Malyugin ring, aid in managing small pupils during surgery.
  • Posterior capsule integrity is crucial for preventing intraocular complications.

Observation:

  • A case report details a 64-year-old monocular patient with a history of diabetic retinopathy and glaucoma.
  • During phacoemulsification, the Malyugin ring migrated posteriorly through a zonular dehiscence, despite an intact posterior capsule.
  • This complication resulted in significant visual disturbance for the patient.

Findings:

  • The posteriorly migrated Malyugin ring was successfully retrieved from the vitreous cavity.

Related Experiment Videos

  • A standard 3-port, 25-gauge vitrectomy was performed for the retrieval.
  • The ring was captured within the barrel of its original injector device for removal.
  • Implications:

    • The original injector device provides controlled extraction of the Malyugin ring from the vitreous.
    • This technique minimizes surgical trauma associated with removing posteriorly migrated iris expansion rings.
    • Effective management of this rare complication is crucial for visual recovery in affected patients.
